JUSTIFICATION OF ORTHOPEDIC TREATMENT OF PATIENTS WITH INCLUDED DEFECTS IN THE LATERAL PARTS OF THE DENTITION

Relevance. Despite the rapid development of dentistry as a science, the popularization and widespread use of preventive measures, tooth loss due to caries remains the main disease, the consequences of which are aimed at overcoming the efforts of modern orthopedic dentists. According to the scientific literature, the number of such patients in need of orthopedic care from the total population of Ukraine ranges from 70.0% to 95.0%. Partial loss of teeth remains one of the most common pathologies of the dentition. Of particular clinical importance are the included defects of the dentition, localized in the lateral sections, since these areas carry the main chewing function and load, so the topic of the study is relevant, and an important task of orthopedic treatment of patients is to restore the effectiveness of chewing for tooth defects in the lateral parts of the lower jaw. Purpose of the work. Systematization and analysis of scientific literature sources on orthopedic treatment of patients with included defects in the lateral parts of the dentition. Materials and methods. The search and systematic review of literature sources related to the purpose of the study was carried out using the databases PubMed, Scopus, Web of Science and Google Scholar. Scientific articles, systematic reviews, clinical trials, meta-analyses that highlighted the relevance of the study were taken into account. Results. According to the results of the analysis, it was found that partial loss of teeth remains one of the most common pathologies of the dentoalveolar system. Of particular clinical importance are the included defects of the dentition localized in the lateral parts, since it is these areas that carry the main chewing function and load. The included defects are characterized by the presence of tooth support on both sides of the defect, which creates, on the one hand, favorable conditions for the use of fixed orthopedic structures, and on the other hand, creates complex biomechanical conditions for the functioning of prostheses. Violation of the integrity of the dentition in the lateral parts leads to the following changes: redistribution of the chewing load; overload of individual teeth; changes in occlusive relationships; development of pathological processes in the periodontium; decreased chewing efficiency. Conclusions. The analysis of literature sources and clinical studies allows us to conclude that the included defects of the lateral parts of the teeth are accompanied by significant changes in the functional state of the dentition system and require a professional orthopedic approach to the choice of prosthetics method. Violation of the integrity of the teeth leads to a change in occlusal relationships, redistribution of chewing load and the development of secondary deformities and defects in the dentition. Timely and rational orthopedic treatment is the most important factor in preventing complications, and further study of the problem of diagnosis and treatment of patients with included defects in the lateral parts of the dentition is relevant and promising.
